1. VOTER's REGISTRATION RECORD (VRR) of parents and/or applicant - from Commission on Elections (COMELEC):

The student-applicant must be bonafide resident of Batangas City for at least one (1) year from the date of application.

2. REPORT CARD:

If graduating: submit a photocopy of Senior High School Report Card (last grading period) with dry
seal, and authenticated by the principal.
If senior high school graduate: Original copy and photocopy of Report Card
If old curriculum: Original copy and photocopy of Report Card
If ALS graduate: Certificate of Rating (COR) and AF-5 form
If transferee: Transcript of Records (TOR)

3. PROOF OF INCOME:

The student applicant and his/her parents must be classified as indigents as proven by:
a. CERTFICATE OF INDIGENCY
b. CERTIFICATE OF RESIDENCY

4. CERTIFICATE OF GOOD MORAL: From the last school attended with DRY SEAL.
5. PSA AUTHENTICATED BIRTH CERTIFICATE
6. ACCOMPLISHED APPLICATION FORM
7. TWO (2) COPIES of DIGITAL 2X2 PICTURE
